Role-Play Script
SW: Good afternoon, Maria. I am glad to see you in the office. How are you and the boys?
Maria: We are alright. Tired, maybe. But we are here.
SW: I would rather you were there even when you were not in fine shape. This reveals how committed you are to your family. I would take time to briefly state the goals we wish to discuss today and the ways we can work together in accomplishing our goals. Does that sound acceptable or not?
Maria: Well, that would be helpful, I guess.
SW: Excellent. Once again, similar to previous sessions, we keep discussions confidential unless there is an emergency that concerns our safety. The steps to be taken in the process of the decision-making process are yours; I do not mediate in your place, and no one dictates or prescribes.
Discovering Maria View and Continuities of Previous Sessions.
SW: During our last session, you listed three very serious concerns, namely the behavior at school of Jose, health issues with Ryan, and your own stress with finances and parenting. I would like to begin by inquiring which of these things has impressed you most since we last met? What has been distracting your mind?
Maria: Mostly Jose. The school called again. He pushed a boy during recess. He must get counseling and maybe medicine again, they say. I do not know. I am scared of all that.
SW: [Reflecting], you know, I am overwhelmed and don’t understand what the school suggests.
Maria: Yes. I would not want them to believe that I am a bad mamma.
SW: [Reframing gently] Maria, I believe your worries are based on your desire to support your son and knowing what is really good for his wellbeing, but not because you think that you are a bad mother.
Maria: I hope so.
Fishing after Meaning, Patterns, and Sequences.
SW: I want to know what the antecedents of Jose’s anger are. What then happens prior to his pressing or fighting? Have you observed a pattern?
Maria: According to him, other children joke around with him. Or that they do not let him play. And then he gets mad fast.
SW: And when he gets angry, what do the other kids do? How does the school respond?
Maria: They tell him that he has to relax. But, according to him, nobody listens. Then it worsens.
SW: This sequence appears to follow a provocation (teasing), the inability of Jose to listen, and later exacerbation, which worsens the situation.
Maria: Yes, that is exactly it.
SW: We can use this pattern to model interventions that will help him build more confidence and learn to manage his strong emotions prior to their emergence.
Eliciting Client Story & Clarifying Meaning
SW: Maria, what would you like to see Jose change in the next few months?
Maria: I would want him to be more at peace, happy, and have friends.
SW: Ambitious goals. What would a content and calm Jose look like compared to what he is now? Other than decreasing of the anger responses, what other behaviors can he display?
Maria: Could we speak more? Come, come, tell me what has happened, yell and go off.
SW: Effective communication, increased emotion, and reduced violent behavior?
Maria: Yes.
Transition to Goal Setting
SW: It means that under your supervision, I would like to develop with you a treatment plan. This will entail laying down broad objectives that will capture the targeted changes and specialized objectives that will form small steps towards meeting these broad objectives. You will still be ahead of this process. Let us begin with Jose.
Goal 1: Improve the Behavioral/Emotional Control in Jose.
SW: Do you feel that the main goal of your work with Jose is to help improve his anger and to express his emotions in a healthy way?
Maria: That is the thing most.
SW: The overall goal can be broken down into smaller ones; that is, two coping techniques, such as deep-breathing exercises or verbal self-regulations, will be taught to Jose to enable him with the means to control frustration. Weekly counseling sessions will be implemented so as to train Jose on the emotional skills and self-confidence; daily ten-minute checks between Maria and Jose will be established to provide a platform to address emotional states. Do these suggestions complete for you?
Maria: Counseling… maybe. I am nervous about it. But the rest sounds okay.
SW: You want to help him, but you have some doubts about the reliability of professionals.
Maria: Yes.
SW: Counseling is not obligatory in the process of giving up control; instead, it is an augmentation of support. Participation on your part will be a critical factor everywhere.
Maria: Okay. Maybe we can try.
Goal 2: Meet the Health needs of Ryan.
SW: Let us now consider Ryan. What do you consider the most important issue in his health?
Maria: His breathing. Some nights it gets scary.
SW: Easily comprehensible. A proper objective would be to enhance the management of his heart condition and asthma. The proposed goals are:
- To make sure that a physician visits the family regularly.
- To find a relatively affordable pediatric cardiologist.
- To obtain financial aid programs that would pay the bills for the medication and treatment. Do these seem beneficial?
Maria: Yes, very much.
Goal 3: Increased Stress Management and Stabilization of Maria.
SW: You have stated that you are overwhelmed and stressed, specifically related to parenting and work-related matters; it will be reasonable to establish the goal aimed at improving your personal health since psychological stability is the key to family well-being.
Maria: I believe so, that is reasonable.
SW: The following goals can be narrated:
- Applying to employment assistance programs.
- Investigation of the caregiver support groups as a stress-reduction tool.
- Plan the home, school, and medical activities every week. What do you find to be the salient one of these?
Maria: Job programs and, most likely, the support group. I do not literally interact with the other adults.
SW: These are good choices. There is empirical evidence that social support leads to perceived stress and feelings of isolation.
Checking for Accuracy and Summarizing
SW: I would now like to sum up the results of our talk. What you would like to see is Jose be calmer, more expressive, and less reactive using coping skills, counseling, and regular interaction at home. With regards to Ryan, you are concerned that he should be given a regular check-up and an allowance. In your own case, you are looking at job-search programs and support groups. Is this what I have gotten down?
Maria: Yes, that sounds good. This fact is what makes it seem possible, since it is possible to look this way.
Finalizing & Booking Next Visit
SW: I compliment you on the improvement you have made today. You have been open, philosophically thoughtful, and true. The next meeting will focus on how these plans are going to be implemented and how effectively they will be. Do you find Wednesday at 2:00 p.m. comfortable?
Maria: That works.
SW: Excellent. Thank you, Maria. You are confidently walking in the interest of your family, and you are doing so successfully.
Maria: Thank you, thank you.
