I have completed 50 practicum hours in the Cleveland Clinic, outpatient internal medicine, and behavioral health collaborative care setting. This practicum location was an experience that provided me with an opportunity to serve a diverse adult community with acute and chronic medical and mental health issues. The hours were completed under the supervision of a certified preceptor and were not towards any training contract.

During my practicum, I have conducted a comprehensive patient examination, including a full history, physical examination, mental health, and drug review. I was also involved in managing chronic conditions that involved hypertension, type 2 diabetes, hyperlipidemia, anxiety disorders, and major depressive disorder. Together with my preceptor, I managed to generate differentials, as well as help with evidence-based treatment plans that could serve the most current clinical methods and the personal factors of the patient.

A large portion of my practicum experience was connected to the concept of behavioral health integration in primary care. I also performed depression and anxiety screenings with the assistance of the standardized tools, assessed the medication adherence, and provided advice to the patients on stress management, lifestyle modification, and coping skills. I had the opportunity to practice in assessing the laboratory results, diagnostic research, and treatment outcomes to evaluate patient progress.

To improve my skills, I used the practicum to talk to my preceptor about clinical reasoning, assessment techniques, and clinical communication skills.
