Achieving Program Outcome 1: Holistic, Patient-Centered Care
Throughout this course, my strengths have been cultivated through assignments and activities that aim at the generation of high-quality patient-centered care with reference to the concepts of holistic health. To give an example, case study analyses required me not only to examine my patients physiologically, but also examine them psychologically, socially, culturally, and spiritually with regard to their health.
Activities like the development of an individual care plan and the creation of patient education strategies enabled me to employ holistic assessment and ensure that all forms of intervention satisfy all forms of patient needs. The interprofessional collaboration activities, like discussions and simulation assisted me to develop my knowledge regarding the effective coordination of care across disciplines without excluding the patient and the family in the decision-making process. The experiences concern directly Program Outcome 1 in that they foster holistic and safe culturally sensitive care practices.
Achieving an Advanced-Level Nursing Education Competency (AACN Essentials Domain 1: 1.2f)
The nursing competencies that I focused on as a part of the advanced course activities include 1.2f: Nursing and other disciplinary knowledge, Synthesize and apply this knowledge to guide education, practice, and research. The assignments that made me critically analyze the research in nursing, medicine, psychology, and social science involved evidence-based practice projects and literature appraisals. It is the combination of the results of both disciplines that allowed me to identify the best practices and turn the evidence into practical intervention and anticipate the potential barriers to patient care.
Based on the theoretical analysis of the strategies to help control the chronic diseases, I incorporated the social determinants of health, pharmacologic, and behavioral health into my recommendations using the example of the strategies analysis. The experience assisted me in becoming more effective in synthesizing knowledge in a systematic manner and justifying it in a patient-centered, multi-disciplinary-based clinical decision-making.
Achieving a NONPF Independent Practice Competency
The course also helped me develop my competency in addressing the competency of the NONPF Independent Practice: The functions of a licensed independent practitioner. I was able to practice independent clinical decision-making without the burden of outcome responsibility with the assistance of assignments in which I was required to plan care, reason diagnostically, and formulate interventions for the diagnosed and non-diagnosed patients. Among others, simulation-based patient encounters and case presentations helped me to learn that I should keep the cultural differences, spirituality, and patient preferences in the care plan so that the patient would be entirely responsible for the decision.
Moreover, the tasks assigned to transitional care and collaboration allowed me to realize that I had an opportunity to organize care throughout the environment and cooperate with other caregivers to reach the greatest benefits. The practices have assisted me in reasserting my knowledge of independent practice value, professional accountability, and patient-centered care value, and prepared me to apply evidence-based practice in clinical practice.
Professionalism in Communication
I was careful to make sure that the written communication in all written assignments and discussions was effective, accurate, and professional. Proper grammar, syntax, punctuation, and spelling of the English language also assisted me in getting to articulate the thoughts in my submissions and demonstrating academic and professional ability. My peer discussions and formal reflection writing helped me to acquire skills in discussing more complex clinical concepts in form and professionally, which is required to work in a team-based environment, educate patients, and share knowledge in advanced nursing practice.
