Amelia Arlington is a 4-week-old baby who presented to Dr. Nayar’s office because she cries a lot. Her parents are worried because she has been fussy since she was born. They are especially concerned about how she eats and how much she cries. Our job as doctors is to check Amelia’s symptoms, give her a thorough check-up, and help her parents with advice and support as they learn to take care of her. We want to examine why Amelia is upset and make sure she stays healthy and grows well.

Diagnosis and Supporting Rationale 

Amelia has been diagnosed with colic. Babies suffering from colic are always crying and are irritated for no clear reason, especially in the first few months of life (Riezzo et al., 2023). 

Rationale

Babies with colic might show signs like pulling up their legs, squeezing their fists, and seeming like they are hurting when they cry a lot (Riezzo et al., 2023). This matches what Amelia is going through, especially in the late afternoon and evening, which has been happening for about a week or so. Also, when doctors checked Amelia, they did not find anything wrong during her check-up, and she is still gaining weight like she should. This helps confirm that she probably has colic.

Pyloric Stenosis

Amelia might have pyloric stenosis, a problem where a muscle in the stomach gets too thick, making babies throw up a lot and feel a lump in their belly that looks like an olive (Plessi et al., 2021). However, since Amelia was not vomiting and doctors could not feel any strange lump in her belly, they decided she probably did not have pyloric stenosis.

Intussusception

3. Environmental Modifications: It is also essential to provide Amelia with a quiet and peaceful environment to help her stay calm (Riezzo et al., 2023). 
4. Follow-up and Monitoring: Amelia’s parents need to bring her more frequently for follow-up appointments to monitor Amelia’s progress and alter the treatment plan as needed (Riezzo et al., 2023).
